CLC’s Common Child Life Internship Application is designed to meet internship programs' needs for obtaining basic applicant information, while simplifying the application process for students. Rather than completing background information multiple times, applicants need only complete one form with the basics of their academic, prior experience, and child life interest information.
The Common Child Life Internship Application is available for applicants to download in both Word and Adobe Acrobat (PDF) formats and may either be saved and updated on a personal computer, or printed and completed by hand. Applicants, please review the below list of things to consider before completing this application.
Internship programs may choose to create a link to CLC’s application webpage to make it easier for students to find the application from the hospital’s website.
CLC encourages all internship programs to accept CLC’s Common Child Life Internship Application Form for basic applicant information. Individual internship programs may also request additional materials as part of their application requirements, including, but not limited to: official transcripts; letters of reference; background check forms; completed coursework reviews; and/or additional essay questions.
As supplements to the Common Child Life Internship Application, there two forms for verifying coursework or practicums in progress. These forms are to be used internship applications only and will be not be accepted for certification purposes. We recommend that students verify that each internship site to which they are applying accepts these forms prior to submitting an application.
